By checking "I Agree," I acknowledge that team members from Conejo Valley Healing Rooms have voluntarily agreed to pray for me. I understand that this session is not a professional counseling meeting and that none of the team members are licensed counselors. I understand that these team members are, to the best of their ability, doing what they can to help me achieve more freedom in my life.
I understand that Conejo Valley Healing Rooms is a California nonprofit corporation and receives a suggested donation for their services. I further state that I have voluntarily sought assistance of my own initiative and that I am under no obligation to accept or reject any of the counsel or help that I might receive from the team members of this ministry.
Conejo Valley Healing Rooms team members offer biblically-based spiritual services to those who desire them. I understand that, if I receive ministry from Conejo Valley Healing Rooms, the team is committed to respect the disclosed information, and these are confidential sessions unless information is disclosed that would alert team members to potential harm to me, the client, or to others. Conejo Valley Healing Rooms observes mandated reporting practices, is a mandated reporter, and is obligated by law to report abuse.
I agree to hold Conejo Valley Healing Rooms and its team members harmless from any and all liability, loss, or damage of any kind that may arise as a result of assistance that I have received, or from my involvement with, Conejo Valley Healing Rooms.
